Living with a Chronic Condition in Latrobe?
We’re Listening

The Latrobe Health Assembly, in partnership with Latrobe Regional Health, Latrobe Community Health Service and Gippsland Primary Health Network have developed the Latrobe Chronic Disease Action Plan 2022-2027. The plan aims to act on a range of areas that will improve the experience of people living with chronic conditions, including early detection and identification, self-management and community-based treatment and care.

To further inform actions within the plan, community insights are being gathered via a five-minute survey. Anyone from the Latrobe Valley who lives with a chronic condition, or cares for someone who does, is invited to share their experience. They are also able to register to become a part of Community Voices for Chronic Conditions, a group of people with lived experience who will inform this work over the next few years.

To thank community for taking part in the survey, they’ll be entered into a draw to win one of four $25 gift cards.

The plan is a direct result of various consultations, community forums and workshops with both service providers, consumers and carers to ensure it meets the specific needs of people living within Latrobe Valley. Providing the community with the opportunity to help shape the plan gives them control over their own healthcare.
