Community Based model of Healthcare for Chronic Disease
Project summary:
The Latrobe Chronic Disease Action Plan 2022-2027 contains several actions that focus on establishing a community-based model of healthcare for chronic disease.
The action plan suggests that the model uses alternative workforces for delivery, considers alternative locations for delivery (outside a general practice clinic) and that it focuses on early detection of disease via health and wellbeing checks.
The Latrobe Health Assembly has engaged Ninety-Mile Consulting to carry out an in-depth scoping exercise of past and existing models. The scoping exercise will include local stakeholder consultation, community consultation, and desktop review to discover learnings from other models and establish a current gap in the local system for a new model.
Following the scoping exercise Ninety-Mile consulting will work with community to codesign a model for trial across Latrobe to which will be presented to the Latrobe Health Assembly board for endorsement.
