Practitioner completion of care plans
Project summary:
Community-based treatment and care of chronic conditions involves multidisciplinary services providing active and ongoing support for people to live their everyday lives, partnering with them to improve health outcomes. To work towards a more integrated and seamless care experience, the Latrobe Health Assembly is working with Meggetto Consulting to review the existing care plan systems that are in place locally and afar, and to develop a set of recommendations on how to strengthen the use of these resources.
This scoping activity informs action 3.2.2 of the Latrobe Chronic Disease Action Plan, building on previous community consultations and data collection. An international literature review has been conducted, and consumers and service providers have been interviewed in order to assess the usage of care plans within the Latrobe Valley.
The final report and recommendations are due to be completed by July 2024. Preliminary recommendation ideas involve increasing capacity of care providers to complete care plans while utilising existing funding and technology systems to improve consumer and service provider outcomes. If implemented, these recommendations are likely to have a beneficial impact upon a range of other activities within the plan, improving continuity of care for community-based services.
